목적 : 인터벤션실에서의 Chemoport insertion 시술 시, 시술자는 환자의 Supine position에서 Catheter tip 의 위치를 결정한다. 이에 본 연구팀은 Chemoport의 적절한 Catheter tip의 위치에 대하여 알아보았다.
대상 및 방법 : 2017년 1월 1일부터 2월1일까지 본원에서 Chemoport 시술을 한 54명의 환자를 대상으로, 인터벤션 투시장치(Artis Q Ceiling, Siemens, Germany)와 일반촬영장치(Digital Diagnost, Philips, Netherlands)를 사용하여, 환자의 Supine position에서 얻은 인터벤션 투시영상과 시술 후의 Standing position에서 얻은 일반촬영 Chest PA 영상을 비교 분석하였다.
결과 : 총54명의 환자에서 Catheter tip의 위치가 Supine position 보다 Standing position에서 Superior vena cava(SVC) 방향으로 평균 16mm 위로 이동하였다.
결론 : 여러 논문들의 보고에 따르면, Chemoport의 Catheter tip의 위치는 SVC-RA junction(cavoatrial junction)에 놓이는 것이 이상적이고, SVC 보다 RA에 위치 하였을 때, 부작용이 더 적다. 그러므로 방사선사는 Chemoport 시술 시, Supine position보다 Standing position에서 환자의 Catheter tip이 SVC방향으로 평균 16mm 위로 이동한다는 점을 숙지하여, 시술자가 Catheter tip의 위치를 Right atrium(RA)방향으로 적절하게 위치 시킬 수 있도록 유도하여야 한다.
