작년 11월 일회용 주사기를 재사용하여 C형 간염이 확산되었다는 사건에 대한 언론보도를 계기로 올해 5월 일회용주사기 재사용 금지 의료법이 국회를 통과하였다. 이제 더욱의료 관련 감염의 중요성이 부각되고 있다. 의료 관련 감염(healthcare-associated infection)이라는 용어는 의료행위와관련되어 발생되는 감염으로 정의하고 있고[1-4], 실제적으로 의인성 감염(iatrogenic infectious diseases)을 이제는 의료 관련 감염에 포함하여 연구하고 있으므로 본 논문에서는의인성 감염을 의료 관련 감염에 포함시켜 기술하고자 한다. 감염학계에서 그리고 정부와 세계보건기구에서도 이미 의료관련 감염으로 의인성 감염을 포함하여 용어를 사용하고 있으므로 의료 관련 감염의 예방과 치료에 대하여 논의하겠다. 의료 관련 감염은 원래는 병원에 입원하고 48시간 지나서 발생한 감염으로 정의하였으나 점차 의미가 확산되어대상을 입원환자는 물론 외래환자를 포함하고 병원에 근무하는 직원, 의료종사자도 아우르게 되었다. 입원하였다가퇴원한 환자가 입원 시 획득된 미생물에 의해 발병된 감염도 같은 범주에 넣어 치유하게 되어 있다. 우리나라에서 의료 관련 감염관리 현황에 대한 연구는 대한의료관련감염관리학회에서 시작하였다[1,3]. 국제적으로 1975-1976년 원내감염조절효과연구(SENIC project, Study on the Efficacy of Nosocomical Infection Control) 자료를 살펴보면 3,770만 입원환자에서 210만 례의원내감염이 미국 6,449(급성치료)병원에서 발생하여 100입원에서 5.7감염의 비율로 발생하는 것으로 이해할 수 있다[5-7]. 현재는 세계적으로 원내감염에 취약한 면역저하환자가 과거보다 증가하여 대략 10% 이내로 발생률을 추정하고 있다[1-4]. 여러 가지 연구의 결과로 감염관리를 하면 의료 관련 감염을 30% 감소시킬 수 있음이 밝혀졌고, 손 위생을 비롯하여미연에 방지하는 방안(Table 1), 즉, 의료 관련 감염관리를위한 표준주의 등이 개발되어 의료 관련 감염을 예방하고 치료하고 있음에도 불구하고[8-11], 경제적 부담, 의료시설의개선 등 개인적으로 실행하기에는 해결할 수 없는 여러 난제는 사회적 과제로 등장하여 국가가 의료 관련 감염에 대한대책도 마련하여 시행하고 있다[3-6]. 본 논문에서는 의료관련 감염의 주요 원인인 내성세균과 함께 의료 관련 감염의4대 과제인 폐렴, 요로감염, 창상감염, 그리고 혈관장치 관련 혈류감염의 예방과 치료를 정리하려 한다.
