Horizontal root fracture is difficult to diagnose in the early stages, and the prognosis is very poor when it occurs at the cervical level. In particular, such trauma occurring in the anterior teeth during adolescence can lead to tooth loss, and there are various limitations in placing implants, which are one of the treatment options. The left central incisor of an adolescent who suffered trauma 6 months ago underwent conventional root canal treatment, and the pain did not decrease, so the patient visited the dental university hospital. This maxillary central incisor had a very poor prognosis due to pulpal necrosis of both coronal and apical fragments and inflammatory reaction at the fractured line.
Considering the patient’s age, the tooth was maintained through endodontic surgery. Maintaining the maxillary incisors during the growing adolescence can prevent tooth loss, provide psychological and physical stability, and harmonize with the growth of the facial features and adjacent anterior teeth and periodontium. The purpose of this report is to discuss a case of maxillary central incisor preservation in adolescent patients for whom immediate implant placement is not possible.
