본 연구는 일반 병동간호사의 정신질환자에 대한 태도, 정신간호역량, 정신질환자 간호업무스트레스의 정도와 차이를 확인하고, 소진과 관련된 영향요인을 확인하기 위한 횡단적 서술적 조사연구이다. 2020년 1월 17일 부터2월 25일까지 서울시 소재 600병상 이상의 종합병원 3곳에 근무하는 임상 경력 1년 이상의 일반 병동간호사(내.외과계 병동, 내.외과계 중환자실,응급실, 간호간병통합서비스) 간호사 186명을 대상으로 실시하였다. 수집된자료는 SPSS 21.0 프로그램을 이용하여 분석하였고, 각 변수에 대한 기술통계와 변수간의 관련성을 파악하기 위해 t-test, 일원배치분산(ANOVA),피어슨 상관관계(Pearson’s Correlation Coefficient) 및 다중회귀 분석(Multiple Linear Regression Analysis)을 실시하였다.
This is a cross-sectional and descriptive study to identify the attitude toward the mentally illness patients, psychiatric nursing competency, and the degree and difference of nursing work stress of mental illness patient in general wards and to determine the factors influencing their Burnout. This study conducted a survey targeting 186 nurses in general wards (medical and surgical wards, medical and surgical intensive care units, emergency rooms, nursing care integration service) with more than 1 year of clinical experience working at three general hospitals equipped with more than 600 beds in Seoul from January 17 to February 25, 2020. The data collected from the survey were analyzed using SPSS 21.0 program, and t-test, one-way ANOVA, Pearson''s Correlation Coefficient, and Multiple Linear Regression Analysis were performed to understand the correlation between descriptive statistics and variables. The results of this study are as follows; The average burnout score of the nurses working in general wards was 4.02 points (±0.67) on a 7-point scale. The average score of the sub-areas of the burnout was in the order of emotional depletion 4.41 points (±1.05), dehumanization 3.83 points (±1.12), and sense of Self-fulfillment 3.68 points (±0.90). The attitude toward the mentally ill patients was 2.74 points (±0.39) on the 4-point scale. The average score of the sub-areas of the attitude was in the order of optimism 3.00 points (±0.43) and stereotype 2.60 points(±0.51). The average score of the psychiatric nursing competency of the nurses in general wards was 2.87 points (±0.46) on a 5-point scale. The average score of the sub-areas of the psychiatric nursing competency was in the order of assessment competency 3.07 points (±0.60), intervention competency 2.80 points (±0.54), resource utilization competency 2.72 points (±0.66), and recommendation for use of psychoactive drugs 2.57 points (±0.86). The nursing work stress of mental illness patient was 3.30 points (±0.51) on a 5-point scale. The average score of the sub-areas of the mentally ill patients nursing work stress was in the order of tension 3.69 points (±0.55), uncertainty of nursing 3.62 points (±0.80), lack of understanding 3.39 points (±0.53), and time pressure 2.88 points (±0.71). The burnout according to general characteristics of the nurses working in general wards showed significant difference depending on their gender (F=2.84, p=.005), religion(F=-2.16, p=.032), and experience of nursing the mentally ill patients(F=2.55, p=.012). As a result of correlation analysis between the attitude toward the mentally ill patients, psychiatric nursing competency, and nursing work stress of mentally ill patients in general wards and their burnout. the attitude toward the mentally ill patients (r=-.38, p<.001) and psychiatric nursing competency(r=-.28, p<.001) showed statistically significant negative correlation, while the nursing work stress with mentally ill patient (r=.44, p<.001) showed statistically significant positive correlation. In order to identify the factors influencing the burnout of the nurses working in general wards, the attitude toward the mentally ill patients, psychiatric nursing competency, and the mentally ill patients nursing work stress as well as the gender, religion, and the mentally ill patients nursing experiences, which showed differences in general characteristics, were converted into dummy variables, and multiple regression analysis was performed using them as independent variables and the burnout as a dependent variable. The major factors influencing the burnout were in the order of the mentally ill patients nursing work stress (β=.30, p<.001), attitude toward the mentally ill patients (β =-.25, p<.001), religion (β=-.21, p=.001), psychiatric nursing competency (β=-.16, p=.016), mentally ill patients nursing experience (β=.14, p=.023), and gender (β=.14, p=.026 ), and the explanatory power of the regression analysis was statistically significant at 33.5% (F=16.53, p<.001). Based on above results, it was verified that lowering the mentally ill patients nursing work stress, enhancing the mental health nursing competency, and creating positive attitude toward the mentally ill patients of the nurses working in general wards can mitigate their burnout related to nursing of the mentally ill patients. Therefore, it is necessary to develop and apply an education program to lower the mentally ill patients nursing work stress, create the positive attitude toward the mentally ill patients, and enhance psychiatric nursing competency for prevention and mitigation of the burnout of the nurses working in general wards.
