In western countries, waist-to-height ratio (WHtR) has been proposed as a more accurate and practical anthropometric indicator for obesity and to predict the risk factors for metabolic syndrome over BMI and waist circumference methods that have been most widely used as traditional markers. The purpose of this study is to verify the validity of WHtR in the Korean population, where body fat distribution is different from that of the western population, and to propose the applicability of WHtR as an indicator of obesity and a risk factor for metabolic diseases. This study also aims to present WHtR as a possible indicator of early health risks in the Korean population by investigating and analyzing the relationship between WHtR values and metabolic syndrome. This study analyzed 2,961 college students (1,309 male and 1,652 female students) who participated in the Korean National Health and Nutrition Examination Survey (KNHNES) from 2008 to 2014. Using the WHtR cutoff value of 0.5 proposed in the western countries, subjects were divided into two groups: the upper group (WHtR≥0.5) and the lower group (WHtR<0.5). When the risk factors for metabolic diseases between the two groups were compared, all risk factors retained statistical significance. This shows that WHtR could be useful in Korea. The actual WHtR cutoff values that were calculated with the study subjects were 0.5 for the entire subjects group and the male students and 0.51 for female students. Applying the WHtR cutoff value of 0.5 as proposed in overseas countries seems adequate when assessing the risk factors of metabolic and cardiovascular diseases on Korean college students. Moreover, it seems WHtR could be used as an essential anthropometric indicator in understanding metabolic syndrome of Korean college students.
